These forms were only intended for end of life care, in which patients in their last days of life select from innocuous looking checkbox choices on whether they want CPR or other life-saving treatments. They are then kept on file indefinitely. You may have come to the hospital for a routine colonoscopy ten years ago, not thinking too much about the answers to the questions, but are now there for something more serious. This can result in life-saving treatments and interventions being denied, based on the decade old answers given on the POLST form. Action must be taken now to protect your family and yourself.
